
Back to School Forms
Please watch THIS VIDEO and follow the link below to complete all of the necessary forms for each City Academy student. Forms must be completed online via ALMA and Magnus Health. We will not provide hard copies. If you need assistance with accessing these forms, please contact Stephanie Dooley at sdooley@cityacademystl.org.
The required forms MUST be completed by Wednesday, August 20 (the first day of school).

- 25-26 Student Participant Likeness Release Statement
- 25-26 Violin Agreement for 1-6th only
- 25-26 Field Trip Form
- 25-26 Parent/Student Handbook Acknowledgement Forms (Read the Handbook HERE)
In addition:
- Complete Magnus Health at mymagnus.com. If you need help, visit our Magnus Help Site HERE.
- Complete the Free and Reduced Lunch Application (must be filled out by hand. Hard copies will be available at the Back to School Celebration on August 16).
