Required Application Steps

Step 1: Application Form

Application forms can be completed online through our Parent Portal. Once you have completed the application, please send your child’s birth certificate and social security card to Stephanie Dooley at

Step 2: Financial Aid Form

The applicant’s parents must complete a Financial Aid application through our Parent Portal. Parents’ most recent Form W2 (Wage & Tax Statement) and Form 1040 (U.S. Individual Income Tax Return) must be submitted with the form.

Step 3: Release of Information, K-6th

Please send the Release of Information Forms to your child’s current school. Request that a copy of all transcripts and test scores, from kindergarten to present grade, be sent to City Academy. Applications will not be considered complete without this information. This information must be sent to City Academy directly from the school and will not be accepted if submitted by applicant’s family.

Step 4: Assessment and Parent Interview

As part of the application process, your child will complete an assessment at City Academy. This process will allow us to assess your child’s social and academic readiness for our program. Assessments and parent interviews are scheduled on weekday mornings for Early Childhood students. Contact Stephanie Dooley for scheduling.

Step 5: Notification of Decision

All decision letters will be sent out March 10, 2023.
